Transitional ACT

The team offers various community-based rehabilitation and case management services, including assertive outreach, basic living skills training, general support, assessments, transportation, and connections to mental health, medical, and social services. Eligibility for T-ACT requires the person served to be aged 16-25 with a primary diagnosis of severe emotional disorder or severe mental illness. They must have a CAFAS score of 100 or higher, with elevated sub-scores (20 or higher) in at least two of the following areas: mood/emotions, school/work, and behaviors towards others. Additionally, they need a minimum score of 10 in the sub-score areas of caregiver resources, self-harmful behavior, and home.
