I/DD Waiver Case Management

West Virginia’s Intellectual and/or Developmental Disabilities Waiver (IDDW) program provides community-based support, including vital case management services to help individuals coordinate medical, behavioral, and daily living supports. Case managers write person-centered plans, track individual goals, and connect members with authorized local providers.
