Assertive Community Treatment (ACT)

The ACT Team caters to adults aged 18 and above who meet all eligibility criteria for Assertive Community Treatment. Comprising qualified teams, it offers a range of community-based rehabilitation and case management services. These include assertive outreach, case management, basic living skills training, general support services, assessments, transportation, and connections to mental health, medical, and social services. Staff members provide supportive counseling, psychiatric, and nursing care as required, including medication management and deliveries. Crisis services are available 24/7. The ACT Team aims to deliver intensive, community-based services to individuals with severe mental illnesses to prevent psychiatric deterioration or hospitalization and to enhance each client’s functioning, independence, and autonomy.
